Foreign students, doctoral students and employees of the University of Warsaw legally residing in Poland can be vaccinated for free against COVID-19 and obtain a vaccination certificate.
In order to get vaccinated you need an e-referral – an electronic document, issued in a special application for health services. If you have a PESEL number issued before May 2021, you should receive an e-referral automatically. Important note: The “pesel” number on your student card does not entitle you to receive an e-referral if it hasn’t been issued by the district office.
If you do not have a PESEL number, you must obtain an e-referral from your primary health care physician. To do so you will need to provide him/her your ID card or passport. The e- referral is valid for 90 days.
HOW TO GET VACCINATION DATE:
Once you have an e-referral, you can schedule your vaccination in one of the following ways:
- call national hotline 989 (press 1 – the staff will help you choose the nearest vaccination point and date that suits you)
- contact the vaccination point directly – check the map to choose the most convenient spot for you
- register online at: https://pacjent.erejestracja.ezdrowie.gov.pl/auth – to use it, you must have a Trusted Profile (Profil zaufany), confirmed e.g. by the bank where you have your account. Assistance of a Polish-speaking person recommended
- send an SMS saying SzczepimySie to the number 880 333 333
REQUIRED DOCUMENTS:
- e-referral
- passport or other identity card issued by your country of origin
- valid visa or temporary residence card
- COVID-19 pre-vaccination screening form (DOCX ENG)
- in case of second dose or booster, when you have taken previous dose outside Poland: a vaccination certificate translated into Polish by a sworn translator
It is important to use the same identity document at each stage of the vaccination.
VACCINATION CERTIFICATE
In Poland we vaccinate with Pfizer, AstraZeneca and Moderna preparations (2 doses), Johnson&Johnson (1 dose). The certificate is issued after the last dose is taken. If you do not have a Patient’s Online Account, you can ask the nurse at the vaccination point or your GP for a printout.
