Most of the questions are translated and explained by us. You should be very careful when you write about your marital status, colour of your eyes or special marks – there are a lot of options in Polish language.

- In section V. you will have to describe your financial resources. Here are some examples of them:

- In section VI. You have to write about your health insurance:
Health insurance from (name of the insurance company), insurance number….
Ubezpieczenie zdrowotne z (name of the insurance company), numer polisy….
At the end of the application you have to include all attachments:
- 2 copies of passport – 2 kopie paszport
- 4 current, colored photos – 4 aktualne, kolorowe fotografie
- Confirmation of payment the stamp duty – Potwierdzenie uiszczenia opłaty skarbowej
- A confirmation from the university of acceptance for studies – Zaświadczenie jednostki prowadzącej studia o przyjęciu na studia
- Proof of payment of the required tuition/course fee – dowód uiszczenia opłaty za studia
- Documents confirming having health insurance – dokument potwierdzający posiadanie ubezpieczenia zdrowotnego
- Evidence of sufficient funds to cover the costs of living and return travel to the country of origin or residence and the cost of tuition/training course:
– certificate on the limit of credit cards – zaświadczenie o wysokości limitu kart kredytowych
– certificate of awarding the scholarship – zaświadczenie o przyznaniu stypendium
– contract of employment – umowa o pracę
– bank statement – wyciąg z konta bankowego
- Transcript of records – Wykaz ocen
- Confirmation of residence in Poland:
– lease agreement – umowa najmu
– confirmation of residence from the dormitory – potwierdzenie zakwaterowania w akademiku
