Starting 4 May 2026, it is possible to apply for a temporary residence permit with the annotation “Previously under temporary protection”
(CUKR card).

❗Who can apply?
The CUKR residence card is available to individuals who meet all of the following conditions:
1. They must be Ukrainian citizens or family members of a Ukrainian citizen (e.g., a spouse).
2. They must hold a valid PESEL number with UKR status both on the date of application and the date the residence card is issued.
3. They must have held UKR status continuously for at least 365 days prior to submitting the application.
4. They must submit a complete application for the CUKR residence card.
5. Their UKR status must have been active on 4 June 2025.
📝 How to apply for a CUKR residence card
To submit your application, please follow these steps:
- Create an account
• Register a new account in the MOS system (Case Management Module).⚠️ If you used the previous MOS system, you still need to create a new account. - Log in and complete the application
• Log in via the government portal: login.gov.pl
• Fill in the online application form for Ukrainian citizens under temporary protection.
⚠️ Do NOT share your login details with anyone — this may result in a loss of control over your personal data. - Prepare required documents
You will need to upload:
• a recent digital photo
• proof of payment (confirmation or scan):
• 340 PLN – administrative fee for the temporary residence permit
• 100 PLN – residence card issuance fee
⚠️ These are two separate payments and must be made to different bank accounts. - Sign and submit your application
• Sign the application electronically using:
➤ a trusted profile (Profil Zaufany),
➤ a qualified electronic signature, or
➤ a personal signature (if applicable).
After submission, you will receive:
• a confirmation of submission (UPO),
• downloadable versions of your application (PDF and XML).
If your application is approved, the regional governor (voivode) will inform you when your residence card is ready.
📅 Important deadlines
➤ 31 August 2026
You must present a valid passport at your local municipality office (urząd gminy) if:
• your PESEL UKR number was issued based only on a declaration (without a passport), or
• you have obtained a new passport after receiving your PESEL.
This is especially important for minors.
✔️ Updating your data is free and does not change your PESEL number.
❗ If you fail to do this, you will lose your UKR status and temporary protection in Poland.
⚠️ Without a valid PESEL UKR, you cannot apply for a CUKR residence card.
